① For the patient : Can be issued after verifying the ID card.
② For agent : Print and fill out the power of attorney and consent form below.

The certificates of Gangnam JS Hospital can be issued after verifying the patient's identity according to the laws and regulations, and other family members, relatives, and agents need a power of attorney and the corresponding documents when visiting the hospital.
Documents for issuing copies of medical records (when patient consent is available)
Patient
① ① 17 years of age or older
- Patient's identification (National ID, passport, driver's license)
② Minors between 14 and 17 years of age
- Patient's student ID or other documents that can verify his/her identity (family relationship certificate, resident registration, etc, identification card of one relative)
③ Under 14 years of age
- Documents confirming the patient's legal representative (national ID card, family relationship certificate, resident registration, etc.)
Patient's family and relatives
① Patient's identification One copy of the ID (ID issued by a public institution, such as a national ID card, passport, driver's license, etc.)
- Identification of the patient's family or relatives and the patient's handwriting consent with the signature
② Documents proving family or relative relationship.
- One copy of family relationship certificate or resident registration
③ One copy of the patient's written and signed consent to access medical records and power of attorney to issue copies.
- Excludes minors under the age of 14.
Patient’s representative
① Patient's identification One copy of the ID (ID issued by a public institution, such as a national ID card, passport, driver's license, etc.)
- Excludes minors under the age of 17.
② Agent's identification One copy of the ID (ID issued by a public institution, such as a national ID card, passport, driver's license, etc.)

- If the patient is unable to sign his/her own due to disappearance, death, unconsciousness, physician incapacity, serious illness, or injury.
